Parent Information

After filling out the registration form you will receive an automatic confirmation email from us to let you know that your child or children have been successfully registered (make sure to check your spam/promotional email folders). If you do not receive the confirmation email, please email Megan Freisleben at megan@clcfamily.church to confirm that your family has successfully registered for VBS this year.

Liability Release and Consent

I, the parent/guardian of the registered child(ren), give permission for my child(ren) to participate in Vacation Bible School (VBS) at Christian Life Center (CLC). I understand that all reasonable precautions will be taken to ensure the safety and well-being of my child(ren) during VBS activities. However, I acknowledge that participation in any activity involves certain risks, and I agree to release, hold harmless, and indemnify Christian Life Center, its staff, volunteers, and representatives from any and all liability, claims, demands, and causes of action arising out of or related to any injury, illness, or damages that may occur during VBS.

In the event of a medical emergency, I authorize the VBS staff to seek medical treatment for my child(ren) and understand that I will be responsible for any costs incurred. I understand that I will also be contacted as quickly as possible in the event of a medical emergency.

Friend Group Requests: We will do our best to accommodate friend requests for group placements, but we cannot guarantee that all requests will be granted. If you would like to request that children of different grades be placed in the same group, please contact Megan Freisleben at megan@clcfamily.church.

Do you have friends you would like to be placed with? We will do our best to group selected friends together in the same crew, but we cannot promise all requests will be possible.
