^{I/We, the parents or guardians of the child(ren) listed in this registration, acknowledge that all physical activities involve inherent risks. While every precaution is taken for the safety and good health of participants, some activities carry the potential for personal injury. Understanding that all due diligence towards safety will be carried out by Waterford Community Church (WCC) staff and volunteers, I/we agree to waive, indemnify, and hold harmless WCC, its personnel, and Board from and against any loss, damage, or injury including any such injury arising from the negligence of the Church or its representatives suffered by the participant as a result of being part of these activities. I/we understand, agree to, and accept these risks and give permission for the child(ren) listed to participate in all activities at WCC Vacation Bible School including but not limited to indoor and outdoor recreational games, sports, and supervised craft projects.}

^{In the event of an emergency, all efforts will be made to contact me/us until reached. If I/we cannot be reached, I/we authorize WCC staff to seek medical assessment and care for the participant named above by any licensed physician or hospital. I/We acknowledge that any additional medical or transportation costs, including ambulance fees, are my/our sole responsibility.}
