VBS CONSENT, RELEASE AND WAIVER

 

I, the parent or guardian, understand that by registering for this joined VBS (West Wilmington SDA (WW), Life in Christ SDA (LINC), Hope Community SDA Church (HCC)) at West Wilmington SDA Church, I am agreeing to the liability, medical, and photo/image releases as stated below for the minors listed in my online registration. I am at least age 18 and have the legal capacity to authorize this release.

 

Liability Release

Every activity sponsored by this VBS is carefully planned and supervised. However, even with the best planning and precaution, unforeseen events and injuries can occur. I agree to assume and accept all risks and hazards inherent in VBS-related activities. I also release and forever discharge and hold harmless WW/LINC/HCC Churches, their officers, directors, coordinators, and adult advisors from all liability, claims and demands of whatever kind or nature, in law or in which may arise or may hereafter arise from the Participant’s participation in the activity.

 

Medical Consent, Release and Responsibility

I do consent to emergency medical treatment if deemed necessary for my minor child before and while all reasonable efforts are made to contact me or the emergency contact.

If I cannot be reached in an emergency during the VBS program, I hereby give my permission to this VBS leadership or church(es) leadership to call 9-1-1 and/or secure proper medical attention for my child as deemed necessary.

I understand, as a parent or legal guardian, I am responsible for the health care expenses incurred on behalf of my minor child.

Image Release and Waiver.

I understand that church events are often photographed/recorded for publication and public relations (i.e. end-of-year videos, website promo’s, flyers, etc), and I hereby grant permission to this VBS to indefinitely use photos/videos of my child(ren) for these purposes (including, but not limited to print, projection, the internet, etc). I waive my right to inspect and approve such use, but such use must be related solely to this VBS and church (es) and must not identify my child by name. I waive my right to any monetary remuneration and release the church (es) from all claims arising out of said use, including but not limited to claims of libel, and invasion of privacy. I understand that I or my child may decline to be photographed and/or videorecorded at any time.
