Parental Consent for Attendance
I, the undersigned parent or legal guardian, grant permission for my child to attend and participate in Tanglewood Church’s Vacation Bible School, “Rainforest Falls,” taking place June 14–18, 2026. I understand that my child will be involved in various activities including games, crafts, snacks, and group learning experiences. I acknowledge that all reasonable safety precautions will be taken, and I release Tanglewood Church, its staff, and volunteers from liability in the event of injury or illness, except in cases of gross negligence.

Media Release
I grant permission for Tanglewood Church to photograph and/or record video of my child during VBS activities. I understand that these images and recordings may be used for church-related purposes including promotional materials, social media, and publications, and I waive any right to compensation or ownership.
