For all documents:
Youth Registration Form (under 18 years old) For all youth participating in events with St. Paul's UMC, we require an annual release form. This form must be filled out and returned to us before a child can be dropped off.
For all adult volunteers, we require an annual background check, entitled Release Form for Adult Background Check.
