If your child(ren) currently attends Faith Formation classes at St. Mark, we already have a Form A and Form B on file.
If your child(ren) does NOT currently attend Faith Formation classes at St. Mark, we will need a completed Form A and Form B for each child, even volunteers.
Your registration is considered final when all forms and fees are received. Registrations are processed on a first come, first served basis and space is limited.
