Anyone 18 years old or older must have a background check on file with St. Mark Lutheran Church. Background checks must be done every 2 years or sooner.
To complete the background check,
- Print and fill-out the form.
- Return it to the Church Office.
- Please allow at least a week for the background check to clear.
