When registering your child(ren), please:
1) include information about allergies and/or medical concerns for each child you are registering. If there are no allergies or medical concerns, please write 'None'.
2) provide your consent to allow VUMC to use photos and/or videos which include your child(ren) in any printed, digital and social media publications. Your consent is provided by answering 'Yes' or 'No' to the question during online registration (or on your paper registration forms). See 'Photo/video Consent' in documents for more details.
3) bring a white t-shirt for each VBS participant by no later than Tuesday morning for your child(ren) to decorate during VBS. If not able to, no worries, we will be sure to have a T-shirt for your child(ren). Please mark the child's name by the neckline of the T-shirt using a permanent marker.
