The undersigned being the lawful parent and/or guardian of the above child(ren), hereby consent to the participation by the child(ren) in all activities conducted by The Point Church and to the participation of the child(ren) in all events related to said activities.
The Point Church cannot and does not guarantee that my child(ren), dependent(s), or I will not become infected with or exposed to the Coronavirus/Covid 19. The undersigned assumes all risk of injury, sickness, or harm to the child associated with participation in activities at The Point Church and agrees to release, indemnify, defend and forever discharge The Point Church and its staff, employees, and agents of from all liability, claims, demands, damages, costs, expenses, actions and causes of action in respect of death, injury, loss or damage to the child, or by the child, howsoever caused, arising or to arise by reason of or during the child’s participation in activities at The Point Church.
