VBS Photo Release: I give permission for photographs of my child(ren) to be used in publications of Monthalia Methodist Church (including the church website and Facebook pages.) I understand that these photos may be viewed by anyone but NO identifying information (ie: last name, age, address) will be displayed. VBS Liability Waiver: As the parent or legal guardian of the below student(s), I hereby give permission for my child(ren) to participate in the Monthalia Methodist VBS Program. I understand that Monthalia Methodist is a nonprofit charitable institution, which is voluntarily presenting this program for my child(ren), other participants, and the community. I also understand that the program has activities that can involve physical contact with other participants, the ground or equipment, and that there is a resulting risk of physical injury to my child(ren). I have explained these risks and benefits of participating in this program to my child(ren) and my child(ren) is in proper physical condition and has no existing injuries or conditions that could jeopardize his/her safety or health, or the safety or health of the other participants. I therefore release and discharge all liability for any harm or injury suffered directly or indirectly as a result of my child(ren)'s participation in the Monthalia Methodist VBS Program, whether or not resulting from negligence, and I agree not to sue Monthalia Methodist, its representatives, staff, or volunteers on any such claim. I also give permission for the staff, representative, or volunteers of Monthalia Methodist to administer first aid or to seek medical care for my child(ren) during my child(ren)'s participation in the program, including transportation of my child(ren) to a medical facility for additional treatment that appears necessary. I acknowledge I have read understand and consent to the above information

Shirts are a crew neck Gildan Softstyle 100% cotton shirt in the color natural.
