Waive and Consent

I hereby authorize Logos Baptist Church (Mississauga) and all camp staff and volunteers to seek necessary medical aid that maybe required as a result of any accident or injury sustained by my child, including hospitalization / transportation if necessary.  In all instances, attempts will be made to contact the Parent/Guardian first.  The participant is responsible for his/her personal health, medical, dental, and accident insurance coverage.  I agree that Logos Baptist Church (Mississauga) and all camp staff and volunteers shall not be liable for any accident or injury sustained by my child or for any loss or damage to personal property arising from, or in any way resulting from, participation in this program. I give Logos Baptist Church (Mississauga) permission to photograph and videotape my child during the camps for non-profit promotional purposes in the future. I also give Logos Baptist Church (Mississauga) permission to share any photograph or video recording, which might include my child during the camps, with other participants of the camp upon request.
