Participant Authorizations
Please read the following information carefully before completing the online Participant Registration process.
When registering your child for VBS at Generations Church, you are agreeing to the following:
- I understand that in the event medical treatment is required, every effort will be made to contact me. However, if I cannot be reached, I give my permission to the staff or sponsor to secure the services of a licensed physician to provide the care necessary, including anesthesia, for my child’s well being.
- I will speak with my child/ren, directing them to stay with their crew and follow the rules set by their crew leaders.
- I will not hold Generations Church, its members or employees responsible for any accident or injury that may occur.
- I give my permission for Generations Church to use any pictures of my child/ren from this event on their website, in publications, and/or in advertisements.
