2021 VBS MEDICAL/HOLD HARMLESS

AND PHOTO RELEASE AGREEMENT

MEDICAL RELEASE STATEMENT: The submittal of this VBS electronic registration confirms that I understand all reasonable safety precautions will be taken at all times during the VBS event to protect my child during VBS. I understand the possibility of unforeseen hazards and the inherent possibility of injury. I understand that in the event medical intervention is needed for this child, an attempt will be made to immediately contact the persons listed on this form. In the event that I cannot be reached in an emergency, I hereby give my permission for First Presbyterian Church/Peace Place to call 911 and request emergency services and/or for a church representative to transport the child to a medical emergency or urgent care facility for treatment.

HOLD HARMLESS AND INDEMNIFICATION: I agree to hold First Presbyterian Church/Peace Place, its leaders, employees, and volunteers harmless from any damages, losses, diseases, injuries incurred by my child, and for photos of my child participating in First Presbyterian Church/Peace Place activities, that are not taken or released by First Presbyterian Church/Peace Place staff, and/or volunteers.

PHOTO PERMISSION RELEASE: Your child may be photographed while participating in VBS activities. Any photos taken by First Presbyterian Church/Peace Place would only be used to promote VBS. By submitting this electronic registration for this child, you are authorizing First Presbyterian Church/Peace Place to use VBS pictures on the Church website, church newsletter, bulletin boards or other Church social media sites to promote VBS. We will never reference your child by name or provide specific information on your child. The pictures will only be used by First Presbyterian Church/Peace Place to show the many ways our children and youth can have fun while participating in the VBS program.

By submitting this electronic registration for this child(ren) you are agreeing to the above statement. If you do not want your child’s picture/video used it is your responsibility to ask for a colored band at the time of registration/check-in each day.
