LIABILITY RELEASE: In consideration for being accepted by Marvin Methodist Church for participation in Marvin Children’s Ministry activities, we, (I) being 21 years of age or older, do for myself and on behalf of my child-participant since said child is under 21 years of age, do hereby release, forever discharge and agree to hold harmless Marvin Methodist Church and the staff and directors thereof from any and all liability, claims or demands for personal injury, sickness or death, as well as property damage and expenses of any nature whatsoever which may be incurred by the undersigned and the child participant that occur while said child is participating in a Marvin Methodist Church sponsored activity.
Furthermore, we (I) and on behalf of our (my) child participant hereby assume all risk of personal injury, sickness, death, damage and expense as a result of participation in recreation and activities sponsored by Marvin Methodist Church.
Further, authorization and permission is hereby given to said church staff, directors and adult leaders to furnish necessary transportation, food and lodging for this participant.
The undersigned further hereby agrees to hold harmless and indemnify Marvin Methodist Church, its directors, staff employees, and other adult volunteer leaders, for any liability sustained by said church as the result of the negligent, willful or intentional acts of said participant, including expenses incurred attendant thereto.
We (I), are the parent(s) or legal guardian(s) of this participant, and hereby grant our (my) permission for him/her to participate fully in activity, and hereby give our (my) permission to take participant to a doctor, medical clinic, or hospital and also do hereby authorize medical treatment if deemed necessary for this participant. We (I) assume the responsibility of all medical bills, if any.
Further, should it be necessary for the participant to return home due to medical reasons, disciplinary action or otherwise, we (I) do assume all transportation cost.
PHOTO RELEASE: I grant to Marvin Methodist Church the right to take photographs and/or videos of me and my family. I authorize Marvin Methodist Church, its assigns and transferees to copyright, use and publish the same in print and/or electronically.
I agree that Marvin Methodist Church may use such photographs and/or videos of me and my family, with or without my name, and for any lawful purpose including, for example, such purposes as publicity, illustration, advertising and web content.
I have read and understand the above.
