PHOTO CONSENT
As part of Morris Chapel United Methodist Church Safe Sanctuary Policy, we need consent from parents or guardians regarding the publishing of children's pictures that are taken during VBS or other children's ministry activities.
I grant permission for my child's photo to be used in Morris Chapel United Methodist Church's publications, including website entries. Photo image include displays, printed materials such as brochures/newsletters, videos, digital images, social media, such as those on the church's website. I understand that my child's last name WILL NOT accompany the photo.
