Photo Release- I hereby grant Colosse Baptist Church permission to copyright and use photographs/videos taken at Foundation Friday events of the minor designated below in any manner or form for any purpose lawful at any time. I waive any right that I may have to inspect or approve the finished product or written copy, that may be used in conjunction therewith, or the use to which it may be applied.

Medical Release- I give my permission for the Foundation Fridays staff and volunteers to administer basic first aid to my child (named below) in the event of an injury. I understand that the Foundation Friday staff will contact emergency services in the event of an emergency, I authorize Colosse Baptist Church staff or volunteers to secure medical treatment for my child, including hospitalization, anesthesia, or surgery, if necessary and all expenses will be paid by me. I understand that all attempts will be made to contact the parent/guardian or emergency contact first.

I understand that while reasonable precautions will be taken, participation carries inherent risks of injury or illness.
