Medical Release

In the event of an emergency, I authorize Christ the King Church and its staff, employees, drivers, sponsors, volunteers, and helpers (collectively, “Representatives”) to take any steps they deem necessary to obtain emergency medical care for my child(ren). I release Christ the King Church and its Representatives from any financial liability that may arise from such emergency treatment.

Photography/Media Release

I grant permission for my child to be photographed or videotaped during VBS activities for internal use only, including sharing with other VBS participants, families, and church members. The images will not be used for public advertising.

Hold Harmless and Release of Liability

I hereby agree to hold harmless and release Christ the King Church (CtK Church) from any liability for injury, loss, or damages suffered by myself or my child(ren), except when caused by the gross negligence or intentional wrongdoing of CtK Church. I further agree to hold harmless and indemnify CtK Church for any injury or damage caused by myself or my child(ren).

I affirm that I am the parent or legal guardian of the minor(s) registered under my name and that I have read, understand, and consent to all the terms and conditions set forth in this Release of Liability. I acknowledge that I am agreeing to this form on behalf of myself, my child(ren), and all other legal representatives, heirs, and assigns.

I understand that this Release constitutes a legally binding contract, and that no representations, statements, or inducements other than those written here have been made. I certify that I have read this Release carefully, understand its contents, and agree to it voluntarily and of my own free will.
