Per Diocesan policy, please complete and sign the Permission/Medical Release for Minors Form online for your child(ren) -- each child needs a completed form. A photo release is also on this form -- please be sure to note if you would approve or decline this for each of your children. Please indicate that you will complete this form. I have read, and agree to abide by the policies stated in the 2025-2026 Religious Education Family Handbook, including the student attendance policy (max of 2 absences for Family Faith or 4 absences for Traditional RE).. As a parent, I understand my role in fostering my child(ren)'s religious education, no matter which program I select for my child(ren). We are required by the diocese to provide you with 4 documents that are linked in the Religious Education Family Handbook on page 7.
