By signing up I am consenting to ANC's ability to photograph me during the program to be used for Church promotional purposes. I also understand that the directors will do their best to place me in my desired job but that serving God means going where He calls me. I will uphold ANC's mission to Love God, Love Others and Make Disciples while serving the Lord during ANC's Vacation Bible School Program.

We will meet in the education building for breakfast and devotionals at 8:15am, except for the first day where we will start at 8:00am. We will debrief after kids have gone home in the education building until 1:30pm. All volunteers are expected to be present and participate in the devotional, morning brief and debrief at the end of the day.

Photo Release: I understand that Almaden Neighborhood Church will be taking photos during Vacation Bible School. I approve of the posting of my photos on church sponsored websites (almadenchurch.org and facebook.com) Please note: Please note: Everyone will be photographed during our VBS program. These photos will be used in our slideshow shown only to VBS participants and their families during the week of VBS. Your consent will allow us to also use this content on our website.

Medical Release: (parents must consent for anyone under 18 years old) I understand that every effort will be made to contact me if my child needs emergency treatment. But if impractical to do so, I hereby give my permission to the physician selected by the Almaden Neighborhood Church VBS staff or volunteer to secure the proper treatment, to hospitalize, to order injections, anesthesia, X-ray or surgery for my child or children named above. ANC will not be responsible for administering medication during the VBS program.
