By signing my kids up for ANC's VBS program I understand that Almaden Neighborhood Church will be taking photos during Vacation Bible School. All children will be photographed during our VBS program. These photos will be used in our slideshow shown to VBS participants and their families/friends at our closing VBS celebration on Friday evening. A selection of photos may be used on our website to highlight the program. For the children's safety, no names will be associated with the pictures.

Will you be purchasing a VBS Music CD? If yes and you have more than one child just select yes on one child if you only want one CD. This will help us make an accurate order. Photo Release: By signing my kids up for ANC's VBS program I understand that Almaden Neighborhood Church will be taking photos during Vacation Bible School. For the children's safety, no names will be associated with the pictures. All children will be photographed during our VBS program. These photos will be used in our slideshow shown only to VBS participants and their families/friends during the week of VBS and our closing VBS program. I approve of the use of photos taken of my child(ren) for church use to promote future VBS programs on ANC's websites. No names or details about the children will be associated with the pictures for the children's safety. If you do not consent, We may still use the photograph but your child's face will be blurred out of the picture before posting.

Medical Release: I understand that every effort will be made to contact me if my child needs emergency treatment. But if impractical to do so, I hereby give my permission to the physician selected by the Almaden Neighborhood Church VBS staff or volunteer to secure the proper treatment, to hospitalize, to order injections, anesthesia, X-ray or surgery for my child or children named above. ANC will not be responsible for administering medication during the VBS program.

Minor injuries: I acknowledge that ANC will treat my child for minor injuries. (Bandaid/Ointment, etc..) Parents will be notified immediately if injury requires more than a bandaid or ice pack. Parents will be notified at pick up for any minor injuries. If your child has an allergy please let us know.
