PHOTO/VIDEO RELEASE:

We love to share our Wildwood Church story with others and so you'll see a lot of photos and videos being taken and shared in many ways. We like to share media items recorded in various ministry activities, events, and programs in church-produced materials including, but not limited to, worship guides, posters, brochures, newsletters, and other printed materials, as well as on Social Media outlets including Instagram and Facebook and the church website. Your participation in Wildwood’s VBS/Go Week grants permission for the images or photos of any of those named above to be used in the manner described. If you do not consent to Wildwood using images/photos of those named above, please email melaniemccollum@wildwoodchurch.org.

MEDICAL RELEASE: I hereby acknowledge that it is my desire (for my child) to participate in activities at Wildwood Community Church. My child is voluntarily participating in these activities with knowledge of the dangers involved and hereby agree to accept any and all risks of injury as a result of such participation and transportation. As lawful consideration for permitting me (my child) to participate in such activities, including the transportation to and from such activities, I hereby agree to indemnify and hold harmless (release and discharge) Wildwood Community Church, its officers, employees, activity coordinators, agents and members of the Board of Trustees from any and all causes of actions, claims, suits, or demands I and my heirs, distributees, guardians, legal representatives or assigns now have or may hereafter have for any injury or damages resulting from the negligence or other acts, howsoever caused, by such church officers, employees, activity coordinators, agents or Trustee members, before or during my participation in such activities on and/or away from the church premises, including transportation to and from such activities. It is specifically understood and agreed to by all parties herein that this release is for claims (1) not covered by insurance or (2) for that portion in excess of any limits of insurance coverage. I REALIZE THAT RESPONSIBILITY FOR WEARING PROPER SAFETY EQUIPMENT (WHEN NEEDED) SHALL REST WITH ME (MY CHILD). I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND AN ASSUMPTION OF RISKS AND SIGN IT OF MY OWN FREE WILL. This consent and Release From Liability shall remain effective until revoked in writing and delivered to any officer, employee or agent of Wildwood Community Church.
