I give permission for my child to attend and participate in all events and activities as a part of Trinity Church’s Activate Children’s Ministry VBS program. It is my understanding that Trinity Church will not be held liable in the event of an injury or illness that may occur while attending VBS and that my child is covered under my personal medical insurance. I also authorize VBS staff to obtain and give treatment for my child, while attempting to contact me, for such injury or illness that may occur in my absence and hold them harmless in the exercise of this authority.

*Please read our Registration/Dismissal Policies:

When you bring your child(ren) each day, please come inside and sign them in at the registration table based on your last name. Notify the person at the registration table as to who will be picking up your child(ren) that day and they will make a note of it for dismissal time.

Upon returning for dismissal, we ask that you remain outside, under the church portico/overhang and wait for your child(ren) to be brought out to you. If all the parents form a line, we can make it go as quickly as possible! We will dismiss individual children and children with siblings in 2 different lines, so look for signs! 3 year olds will have their own dismissal as well.

*Please understand that it will take quite a while to dismiss so many children and be patient with us (especially Day 1!) as we dismiss everyone as safely and orderly as possible. Thanks in advance!
