Emergency Transport Policy: We require permission to transport your child to the hospital in the case of a severe or life threatening emergency. We will use all due diligence to contact you prior to transport. In the event you cannot be reached we need your permission to have an ambulance transport your child to the nearest local emergency room. Do you give permission for us to transport your child. Photo Release Policy: Do you give permission for your child to be photographed and or video taped as part of their daily activities at VBS. We sometimes use photos and video to promote our programs both in person and online via our Faceboook Page and YouTube chanel.

Wellness Policy: I understand that my child can not participate in VBS if they have any of following signs of illness: fever, chills, vomiting, diarrhea, rash, head lice. If they present signs of illness at VBS I will be contacted and must pick up my child promptly. Answer by typing the parent/guardian legal name.

Pick Up Policy: I understand that pick up time for all children is at 8pm promptly. If for some reason I will be late it is my responsibility to let the church know. Legally we can not drive your child home. If parents are not present after 8:30pm and the emergency contact person can't be reached we will have no choice but to contact the local authorities. Participation Policy: I acknowledge my child has my permission to participate in VBS (vacation Bible school) at (BHUMC) Brook Hill United Methodist Church. BHUMC will not be liable for any injury sustained, or be responsible for any medical expenses incurred as a result of my child’s participation in this program.
