By registering your child(ren) you are confirming that you are the Parent and/or Guardian. You are also agreeing to terms stated in the Release Agreement, Medical Liability & Release, Dress Code and Code of Conduct.

Release Agreement for 2026 Team Jesus Summer Camp

I agree and release the Word of Faith Church, its volunteers, staff and its employees from any loss, liability, damage or cost incurred, during the WORD OF FAITH CHURCH’S TEAM JESUS SUMMER CAMP (July 20– July 24, 2026; ages 4yrs – 18yrs) and its sponsored activities. whether by the negligence of the church or otherwise.

I and any personal representatives, heirs, and next of kin, hereby releases, waives, discharges and covenants not to sue Word of Faith Church, its volunteers, staff and its employees on account of injury or property damage caused by a sponsored activity.

I and any personal representatives, heirs, and next of kin, hereby assume full responsibility for and risk of bodily injury, death or property damage due to negligence of releases, while attending, participating, competing, officiating, and/or working for any purpose while participating in any sponsored activities.

I and any personal representatives, heirs, and next of kin, further release Word of Faith Church, its volunteers, staff and its employees from any claim whatsoever on account of first aid treatment or service rendered to myself or child during the participation in any sponsored activities.

I and any personal representatives, heirs, and next of kin further agree that I have carefully read the forgoing release and know the contents of the release and sign this release as my own free act.

Photographs, Videos and Recordings

I hereby grant permission to WORD OF FAITH CHURCH, INC. (San Pablo, CA) and its representatives to take photographs or videos of my child and/or children and to make recordings of their voice at the 2026 WORD OF FAITH CHURCH TEAM JESUS SUMMER CAMP.

I further grant to the WORD OF FAITH CHURCH INC. (San Pablo, CA) and their representatives the right to reproduce, use, exhibit, display, broadcast and distribute and create derivative works of these images and recordings in any media now known or later Developed. I acknowledge that WORD OF FAITH CHURCH INC.(San Pablo, CA) owns all rights to the images and recordings.

Waiver, Indemnity and Release

I hereby waive any right to inspect or approve the use of the images or recordings or of any written copy. I further waive all moral rights. I also waive any right to royalties or other compensation arising from or related to the use of the images, recordings, or materials. I hereby release, defend, indemnify and hold harmless the producers from and against any claims, damages or liability arising from or related to the use of the images, recordings or materials, including but not limited to claims of defamation, invasion of privacy, or rights of publicity or copyright infringement, or any misuse, distortion, blurring, alteration, optical illusion or use in composite form that may occur or be produced in taking, processing, reduction or production of the finished product, its publication or distribution. I am the parent and/or guarding of the child/children listed below. I have read this document before signing below, and I fully understand the contents, meaning and impact of this consent, waiver, indemnity and release. This consent, waiver, indemnity and release is binding on me, my heirs, executors, administrators and assigns.

“In the event that I cannot be reached in an emergency during the dates specified on this form, I hereby give my permission to the physician or dentist selected by the church leadership to hospitalize, to secure proper treatment, and/or order in injection, anesthesia, or surgery for my son or daughter as deemed necessary.”

Liability Release

Every activity sponsored by this church is carefully planned and adequately supervised by mature adults. However, even with the best of planning and precaution, unforeseen events can occur. The child(ren)’s Parent/Legal Guardian is responsible for the healthcare decisions of the minor(s). The Parent/Legal Guardian’s medical and/or dental insurance is responsible for the payment of all medical, dental or hospital care. By signing this form, the parent or guardian agrees to assume and accept all risks and hazards inherent by church related social activities. They also agree not to hold this church or its volunteer assistants liable for damages, losses, or injuries to the person or property of the undersigned. The parents or guardians understand that they are signing for the minor listed on this form and the signature is for both a medical and liability release.

Dress Code Agreement

Unacceptable Clothing:
• Any clothing with writing that disrespects or mocks God and/or the Bible.
• Any clothing promoting consumption or companies that produce or promote alcohol, tobacco, or illegal drugs.
• Any clothing that explicitly or implicitly promotes racism, sexism or hatred of any group or person.
• Any clothing that explicitly or implicitly promotes or refers to sexual actions or situations.
• Visible undergarments
• Short shorts (Anything shorter than 3 in. above the knee when standing)
• Short or tight dresses and skirts, including mini-skirts
• Tight pants or shorts, or tight fitting tops
• Strapless, backless, or halter tops
• Sheer blouses or low-cut necklines
• Tank tops or bare mid-riffs

Young Ladies: May wear knee-length shorts (Anything shorter than 3 in. above the knee when standing) slacks or jeans that are not tight.
A dress or skirt is appropriate for the Sunday Service.

Young Men: May wear knee-length shorts (Anything shorter than 3 in above the knee when standing) or jeans that are not tight.
A dress shirt and pants is appropriate for the Sunday service.

Pants should not reveal any sort of undergarments.

Code of Conduct

As a Christian organization, Word of Faith Church expects all students attending TEAM JESUS SUMMER CAMP to conduct themselves appropriately and abide by the church rules and regulations.

While attending WOFC TEAM JESUS SUMMER CAMP, Students must agree to comply with the standards of “The Dress Code” and follow the basic structured rules established by Word of Faith Church.

Please Note: All forms of electronic devices, (Smart phones, Cell phones, I pads, IPods earphones etc…) are prohibited while attending Team Jesus Summer Camp.

Any student found in possession of or using tobacco products, pornographic materials, alcoholic beverages, or any form of illegal drugs will be sent home. Any non-compliant student will also be subject to dismissal.
