Each family has a unique code to identify the person authorized to pick up children after VBS. You can choose a code for your family this year. Your code can be letters, numbers, or a combination. Anyone picking up children will need to provide the code to have children released into their care.

PHOTO, VIDEO & MEDIA RELEASE

From time to time, SSC will create presentations to publicize events or web pages for publication on social media. I give my permission for South Sub Church (SSC) to use photographs, digital or video footage of my child(ren) in social media and other presentations about the Church and its programs and people. All photographs, digital or video footage shall remain the sole property of SSC. I understand that no compensation will be made to me for this use.

All presentations or Web publications including images of children are subject to the following terms: all documents may include only children’s images; no confidential information regarding any child shall be published or linked to a Web page; and no names may be associated with pictures. My child(ren)’s work, photograph(s), or image on social media will only be presented or published with these terms.

If you agree with these terms, children will be photographed for the daily Spotlight Video at Closing and may be photographed for South Sub's social media.

If you disagree your child will not be photographed.

GENERAL MEDICAL PERMISSION & RELEASE

I am voluntarily allowing my child(ren) to participate in SSC’s activities and programs. I waive, release, and agree to indemnify SSC and its volunteers and employees regarding any claims, liabilities, expenses, or negligence of any kind arising out of my child(ren)’s participation in or travel to/from SSC programs and activities. In case of a medical incident involving my child(ren), I understand every effort will be made to contact me. If the incident is believed to be serious, I authorize an SSC representative to call 911 and consent to emergency medical treatment for my child(ren). I understand that I am responsible for any costs incurred on behalf of my child(ren) for such treatment.
